Membership Costs

$250 membership fee/year per individual. Add additional family members for only $150/year per family member.

Discount Plan Exclusions and Policies:

This discount plan cannot be used in conjunction with any insurance plans, discount plans, workers compensation benefits or dental benefits included under an automobile medical insurance claim.

This plan can only be used at AV Dental Associates PA, in Ramsey, NJ.

Plans are non-refundable.
Unused benefits cannot be carried over to the following year.

Patient’s portion for any treatment is due before the treatment is provided.
